There is a telehealth consent form, please sign it even if you don't intend on utilizing telehealth services. This allows us to preemptively take care of any event that may arise in the future. Once you have filled out the forms and signed all required documents, it will give you an option to send it. Once it is sent, you will be required to confirm your email address.
